Moses Taylor Foundation Announces Request for Proposals to Reduce Older Adult Isolation

In January, Moses Taylor Foundation shared its 2019-2023 Strategic Plan. One of the core strategies included in this plan was a new initiative to reduce older adult isolation, a critical social determinant of health. The Foundation spent the first half of 2019 laying the groundwork and gathering information to launch this work and we are pleased to announce the first related funding opportunity: 
 
Reducing Older Adult Isolation Planning Grant RFP
 
This grant opportunity will provide planning funds to a collaborative of organizations working together to create a community system to reduce older adult isolation in Lackawanna County. 
 
The key outcome of the grant will be a plan for implementation of a coordinated system that includes a:
  • Shared isolation screening tool
  • Team of trained navigators to connect isolated older adults to services
  • Corresponding public awareness campaign

A broad array of wrap around supports will be provided to the funded collaborative to enrich the process, including data and evaluation assistance, subject matter expertise, and community input. At the conclusion of the planning period, it is expected that the chosen collaborative will utilize their developed plan to submit a proposal for implementation funding.
